The medicine you took this morning traveled a long route to get from the lab to your pill pack. First, there’s extensive lab research. Then, animal testing. But before a medicine can be approved for use, it must be tested on humans – in an expensive, complex process known as a clinical trial.

The Basics

In its simplest form, a clinical trial goes something like this: Researchers recruit patients who have the disease that the experimental drug is aimed at. Volunteers are randomly divided into two groups. One group gets the experimental drug; the other, called the control group, gets a placebo (a treatment that appears identical to the drug being tested, but has no effect). If the patients who get the active drug show more improvement than the ones who get the placebo, that’s evidence that the drug is effective.

One of the most challenging parts of designing a trial is finding enough volunteers who meet the exact criteria for the study. Doctors may not know about trials that might fit their patients, and patients who are willing to enroll may not have the characteristics needed for a given trial. But artificial intelligence might make that job a lot easier.

Meet Your Twin

Digital twins are computer models that simulate real-world objects or systems. They behave virtually the same way, statistically, as their physical counterparts. NASA used a digital twin of the Apollo 13 spacecraft to help make repairs after an oxygen tank exploded, leaving engineers on Earth scrambling to make repairs from 200,000 miles away.

Given enough data, scientists can make digital twins of people, using machine learning, a type of artificial intelligence in which the programs learn from large amounts of data rather than being specifically programmed for the task at hand. Digital twins of patients in clinical trials are created by training machine-learning models on patient data from previous clinical trials and from individual patient records. The model predicts how the patient’s health would progress during the course of the trial if they were given a placebo, essentially creating a simulated control group for a particular patient.

So here’s how it would work: A person, let’s call her Sally, is assigned to the group that gets the active drug. Sally’s digital twin (the computer model) is in the control group. It predicts what would happen if Sally did not get the treatment. The difference between Sally’s response to the drug and the model’s prediction of Sally’s response if she took the placebo instead would be an estimate of how effective the treatment would be for Sally.

Digital twins are also created for patients in the control group. By comparing the predictions of what would happen to digital twins getting the placebo with the humans who actually got the placebo, researchers can spot any problems in the model and make it more accurate.

Replacing or augmenting control groups with digital twins could help patient volunteers as well as researchers. Most people who join a trial do so hoping to get a new drug that might help them when already approved drugs have failed. But there’s a 50/50 chance they’ll be put into the control group and won’t get the experimental treatment. Replacing control groups with digital twins could mean more people have access to experimental drugs.

The Unexpected

The technology may be promising, but it’s not yet in widespread use – maybe for good reason. Daniel Neill, PhD, is an expert in machine learning, including its applications in health care, at New York University. He points out that machine learning models depend on having lots of data, and it can be difficult to get high quality data on individuals. Information about things like diet and exercise is often self-reported, and people aren’t always honest. They tend to overestimate the amount of exercise they get and underestimate the amount of junk food they eat, he says.

Considering rare adverse events could be a problem, too, he adds. “Most likely, those are things you haven’t modeled for in your control group.” For example, someone could have an unexpected negative reaction to a medication.

But Neill’s biggest concern is that the predictive model reflects what he calls “business as usual.” Say a major unexpected event – something like the COVID-19 pandemic, for example – changes everyone’s behavior patterns, and people get sick. “That’s something that these control models wouldn’t take into account,” he says. Those unanticipated events, not being accounted for in the control group, could skew the outcome of the trial.

Eric Topol, founder and director of the Scripps Research Translational Institute and an expert on using digital technologies in health care, thinks the idea is great, but not yet ready for prime time. “I don’t think clinical trials are going to change in the near term, because this requires multiple layers of data beyond health records, such as a genome sequence, gut microbiome, environmental data, and on and on.” He predicts that it will take years to be able to do large-scale trials using AI, particularly for more than one disease. Gathering enough quality data is a challenge, says Charles Fisher, PhD, founder and CEO of Unlearn.AI, a start-up pioneering digital twins for clinical trials. But, he says, addressing that kind of problem is part of the company’s long-term goals.

Two of the most commonly cited concerns about machine learning models – privacy and bias – are already accounted for, says Fisher. “Privacy is easy. We work only with data that has already been anonymized.”

When it comes to bias, the problem isn’t solved, but it is irrelevant – at least to the outcome of the trial, according to Fisher. A well-documented problem with machine learning tools is that they can be trained on biased data sets – for example, ones that underrepresent a particular group. But, says Fisher, because the trials are randomized, the results are insensitive to bias in the data. The trial measures how the drug being tested affects the people in the trial based on a comparison with the controls, and adjusts the model to more closely match the real controls. So, according to Fisher, even if the choice of subjects for the trial is biased, and the original dataset is biased, “We’re able to design trials so that they are insensitive to that bias.”

Neill doesn’t find this convincing. You can remove bias in a randomized trial in a narrow sense, by adjusting your model to correctly estimate the treatment effect for the study population, but you’ll just reintroduce those biases when you try to generalize beyond the study. Unlearn.AI “is not comparing treated individuals to controls” Neill says. “It’s comparing treated individuals to model-based estimates of what the individual’s outcome would have been if they were in the control group. Any errors in those models or any events they fail to anticipate can lead to systematic biases – that is, over- or under-estimates of the treatment effect.”

But unlearn.AI is forging ahead. It is already working with drug companies to design trials for neurological diseases, such as Alzheimer’s, Parkinson’s, and multiple sclerosis. There is more data on these diseases than on many others, so they were a good place to start. Fisher says the approach could eventually be applied to every disease, substantially shortening the time it takes to bring new drugs to market.

If this technology proves useful, these invisible siblings could benefit patients and researchers alike.

I work as a chief physician in an integrative medicine clinic. As part of my activities, I perform preliminary diagnoses of newly arrived patients and accept several patients daily.

With every patient, we start with a detailed health interview and ask about their current symptoms. Health complaints vary; people have different diagnoses, family health histories, and medications. However, one theme is prevalent: 8 out of 10 patients suffering from chronic fatigue and low energy levels. Some feel weak and sleepy during the day, some must take naps during the day, and others have no energy even after sleep. These are long-term symptoms, often lasting for years. Usually, people mitigate them with multiple supplements and stimulants, but the effects are not last. This theme is so common that it is important to dig down to the roots, understand the real reasons for low energy levels, and offer general non-pharmaceutical solutions. 

Let us start at the beginning. Most of our energy is produced via cellular respiration from the food we eat (carbohydrates, proteins, and fats) in mitochondria in the form of Adenosine Triphosphate (ATP) molecules. This process consists of 4 stages and is very efficient: 38 molecules of ATP are produced from one glucose (basic sugar) molecule. This amount is many times higher than what bacteria or cancer cells produced by glycolysis. Usually, we generate enough energy to carry us through the day without tiredness. Yes, there are some periods of sleepiness, usually shortly after meals and mid-day. These off-peaks are natural, temporary, and well-explained by the metabolic cycle and circadian rhythm. 

Chronic fatigue is a very different story. Let’s consider what can go wrong in energy production for so many people. Quite a few things:

  • There can be insufficient oxygen access in the lungs. Unless you are an advanced practitioner of kriya-yoga, your energy is totally dependent on the efficient consumption of molecular oxygen, O2, by the lung alveoli. Oxygen is bound to hemoglobin in our red blood cells (RBCs, or erythrocytes) and transported throughout all organ systems. We need good unscarred lungs (I hope you don’t smoke or vape) and intact RBCs. This correlation is very straightforward. Patients with a lot of lung fibrous tissue (usually due to age, years of smoking, and lack of exercise) are often chronically tired. The good news is that lung tissues regenerate at any age. Its function is greatly supported by endurance exercises such as walking, jogging, or swimming, breathing programs such as pranayama, energy practices such as Qigong, and other methods. In our clinic, we apply a simple system of breathing exercises that we practice on the cliff over the tide waves (there is an additional benefit of breathing sea water in nano-drops formed by crashing waves).
  • The health of red blood cells (RBCs) could have deteriorated. Erythrocytes are quite fragile and sensitive to environmental stress. During blood microscopy of people with low energy (best seen in 1000X magnification, phase contrast objective), we often see RBCs damaged, misshaped, or aggregated together in long stacks (rouleaux) (Fig.1). Standard blood tests are unable to detect this damage. Typically, patients with chronic fatigue syndrome have a normal count of RBCs, levels of hemoglobin, hematocrit, and free iron, and their doctors are puzzled. In many cases, the morphological damage to RBCs is reversible, and selected IV drips can restore their shape and function in 1-2 days. Sometimes, however, the damaged RBCs are persistent in the blood, and the patients continue to feel tired due to tissue hypoxia. In such cases, the damaged RBCs must be eliminated throughout the system, and a new healthy generation is produced from stem cells in the red bone marrow. We accelerate the process with IV laser therapy to induce stem cells and activate apoptosis, or programmed cell death, to speed the elimination of the damaged RBCs. This process usually takes 7-10 days. We know that it is completed when patients wake up one day with a broad smile and say that the air was remarkably crisp that night. We don’t even need to look at their blood. We know we will see happy unaggregated donut-shaped erythrocytes, and the energy is back.

  • There could be issues with mitochondrial health. Our mitochondria being our main ATP factory, their well-being is key to our energy production level (Fig.2). Virtually all chronic conditions impact the mitochondria. Those cells have their own genome, and their mutations are inheritable, although mitochondrial genetic diseases are rare, affecting some 1 in 5000 individuals. More frequently, mitochondria’s failures to produce enough energy are not hereditary and are associated with acquired conditions such as diabetes, muscular dystrophy, neurodegenerative diseases, high levels of systemic inflammation, cancer, and more. Mitochondria have an electric potential of 160 mV on their membrane, and energy production depends on this charge. Once the membrane loses this charge, energy production drops. There are also many compounds and metabolites inhibiting mitochondria’s enzymes. A remnant of ancient bacteria, the mitochondria are also sensitive to direct signaling by the human microbiome, particularly from the gut. The good news is that damage to mitochondria health is reversible, and to some extent, they can be recharged, just like Tesla batteries. Red light therapy or laser can do this in a few procedures.

There is a potential of 160mV between 2 layers of membrane

  • Our metabolism could be off. ATP is produced by oxidizing pyruvate (coming from carbohydrates), amino acids (from proteins), and fatty acids (fat). The efficiency of this process is directly related to our diet and the ability of our metabolism to fully process food. Our metabolic system is robust, but it can be negatively affected by unhealthy or excessive food intake, disbalances in the “Western diet”, consumption of modified food (for example, food containing hormones, antibiotics, and glyphosates found in many products), irregular meals, etc. Metabolic failures disrupt the “supply chain” for mitochondria energy stations, and, just in the global economy during a covid pandemic, they cause a sharp decline in ATP production. How to avoid it? Well, you know the drill: eat only when you feel hungry, add low-calorie fasting days to your calendar, slow your metabolism by reducing your intake of dead animal protein, and follow the rules of intermittent fasting. In our experience, following these simple rules help immensely.
  • Our microbiome. A former bacterium themselves, mitochondria are part of our global microbiome, a universe of microorganisms that outnumber human cells 100:1 in our own body. As it became recently known, to a large extent, our health, tastes, mood, and even social interactions are dependent on our microbiome composition and communal metabolism. The bad microbes in the gut, intestine, and blood (for example, filamentous fungi, candida yeast, klebsiella, and borrelia) can directly drain our energy, make us eat more sugars, and signal mitochondria to slow down ATP production. Dysbiosis of the gut microbiota and mitochondrial dysfunction are both associated with chronic intestinal inflammation, and this condition greatly diminishes mitochondrial enzymatic processes. How to balance the microbiome? The short answer is to supply gut microbes with their favorite food at the right time. The longer story deserves a separate blog.
  • Systemic chronic inflammation (SCI) could be present in the body. By now, it is well understood that chronic inflammation is the basis for most chronic conditions, from cardiovascular diseases to diabetes and cancer. Mitochondrial dysfunction plays a significant role in establishing the state of SCI via pro-inflammatory signaling. And vice versa, the stage of SCI impairs ATP production by reducing the TCA cycle metabolism and membrane potential.

These are the main factors that regulate our energy production. Deficiency at these levels can severely impact ATP levels and cause chronic fatigue. The good thing, though, is that these deficiencies are mostly reversible. In the short run, red light and laser therapy can recharge the mitochondrial electric potential. Deep blood cleansing and lymphatic system detox decrease the level of systemic information and help to restore enzymatic complexes on the mitochondria membrane. In the long run, changing habits and diet restores a healthy gut microbiome, which can sustainably maintain high energy levels in mitochondria. Physical activity, walking, and breathing exercises help a lot: 30-60 min of physical activity per day substantially increases our energy production.

Hormonal fluctuations associated with menstruation cause the uterine muscles to tighten and compress, resulting in painful menstrual cramps. By contracting and squeezing (imagine your arm biceps bulging), you may expel the menstrual tissue and blood that you usually lose every month during your periods.

Whether or not you use a menstrual cup, cramping in your uterus is a given. Your menstrual cup is nowhere near the cramping, contracting muscles of your uterus. Thus, although period cups are saviors since you may use them longer than tampons and they don’t leak, they are not a panacea for periods discomfort.

Do menstruation cups aggravate cramping?

We doubt it. Pain from menstruation cups is only an issue if the cup is inserted too near the cervix (the mouth or opening to your uterus). Occasionally, a period cup and your cervix might create suction, which can feel like a tugging or painful sensation. Menstrual cramps cause discomfort that aches, throbs, and occasionally wave in intensity, but this is different.

The size, shape, and position of your uterus and cervix will play a role in determining which menstrual cup size is best for you. The likelihood of experiencing pain from your period cup being too near your cervix decreases if you use the appropriate menstruation cup.
